Mandatory Requirements

Client is in need of one Full-time Social Worker, working normal business hours Monday - Friday.  Overtime may be required.  The Social Worker will work together with the Home's Licensed Master Social Worker 2.   A Licensed Master Social Worker is preferred, but we would consider a candidate with a Bachelor of Social Work (BSW)or a candidate that holds an Associate degree in Human Services.

II. Licensed Master Social Worker lIConsultant
 
1. To assess the problems/needs through resident/family interviews, completion of
designated sections of the MDS 3Q and participate in the development of
interdisciplinary care plans; to review and revise the assessed approaches to the patients
as needed to keep them current.
2. To participate in pre-admission interviews for potential residents, as requested.
3. To complete a thorough and detailed admission comprehensive psychosocial assessment
for all new residents assigned.
4. To provide counseling to facilitate adjustment to social, physical, and environmental
changes; to resolve conflict situations using crisis intervention and supportive counseling
techniques. Due to the increased complexity of case mix residents admitted to the
facility, more intervention is required to provide support and counseling.
5. To maintain timely records through routine progress notes and documentation of
significant events.
6. To develop social histories (including trauma care assessment), which reflect
determinants of diagnoses and prognoses toward adjustment for interdisciplinary use.
7. To serve as an advocate of Residents' Rights and enforce the procedure of the Facility
Complaints Policy.
8. To assist in orienting all new employees to the Resident's Bill of Rights, Patient Abuse
Law, Mandatory Report Procedures, and Department of Social Services.
9. To provide information and referral by functioning as a liaison between residents,
families, community, and the facility; in addition, to respond to inquiries from the
community regarding the New York State Home.
10. To deliver business and financial mail to residents as requested.
11. Other duties as assigned.
12. Performs duties and tasks as listed for Social Work Assistant.

III. Social Work Assistant/Consultant
Brief Description
Provides support and assistance to Licensed Master Social Worker 2 and Licensed Master
Worker 1.
Duties and Tasks
1. Coordinate delivery of business, financial and personal mail to residents.
2. Obtain needed personal items for residents.
3. Assist residents on shopping trips as the residents are able and desire.
4. Assist residents in Resident Council Meetings and the completion of the minutes for each
meeting.
5. Assist family members in gathering information and completing paperwork for the Medicaid
application.
6. Assist with the completion of required paperwork including the 10-1 OSH, as directed.
7. Coordinates distribution of deceased resident's belongings upon direction offamily and/or
LMSW.
8. Assist residents in registering and/or apply for absentee ballots to ensure they are able to vote.
9. Coordinate cable and/or phone service for the resident's room, upon request.
10. Ensure each resident receives a key for their locked drawer.
